Job Description
Are you a healthcare professional passionate about patient care? ReGen Rehab Hospital is looking for a dedicated Case Manager to oversee patient care coordination and ensure smooth care transitions.
Key Responsibilities:
- Manage patient journeys from admission to discharge
- Develop comprehensive care plans with healthcare teams
- Support patients and families throughout rehabilitation
- Liaise between medical staff, patients, and insurance providers
- Monitor care plans for optimal outcomes
Qualifications:
- Degree in Medicine, Physiotherapy, or Nursing
- Experience in patient care management
